Island Health has exciting opportunitiesfor Nurse Practitionersto work in the Victoria area at new Urgent and Primary Care Centres, with a team of primary care providers and other allied health staff to collectively deliver integrated team-based care to patients.
In this service contract position the nurse practitioner will work collaboratively with an inter-disciplinary health care team to provide culturally safe urgent and primary care. Patient visits will consist of unscheduled urgent care and scheduled longitudinal full service or same day primary care patient visits.
Acting as the most responsible primary care provider, there is a requirement for the practitioner to establish a minimum patient panel.
This position will provide regular primary care with a focus on mental health and substance use.
After hours’ care for patients is inherent in the services. The nurse practitioner will participate in an after-hours call group, sharing after-hours coverage with other practitioners, to include physicians and nurse practitioners, at the clinic.
